This form is for new students who have never previously enrolled in Trinidad Union Elementary School District (TUSD).

You will be asked to provide information including emergency contacts, medical history, and language background. Once submitted, your application will be electronically sent to the school for review and confirmation.

What You Will Need

Please have the following documents ready before you begin:

1. Proof of birth (birth certificate, passport, or hospital certificate)
2. Proof of residency (In-district students only) — 2 documents required (utility bill for gas, electricity, or water dated within the last 30 days)
3. Immunization records — medical exemptions are not accepted unless submitted to CAIR directly by your child's pediatrician
4. Parent/guardian photo ID
5. Court documents (e.g. restraining orders), if applicable
